How to Make a Patient Referral

To refer a patient, please use ChartLink, call in a referral, or send a fax to 617.638.6756 and include the following information:

  • Your name, practice location, and contact information
  • The patient’s full name, date of birth, contact information, and insurance (if known)
  • The clinic you’re referring to
  • Reason for referral

How to Order Imaging or Labs

Imaging

All Radiology orders, including mammography, should be faxed to 617.562.7851.

If you have questions, call:

If Your Organization Is on BMC’s Epic or OCHIN

If you’re a BMC Health System provider submitting an order for a BMCHS patient, or you work for an organization that uses OCHIN or BMC’s Epic (such as certain Boston HealthNet CHCs), you can order lab tests directly through Epic. Just search for the lab or imaging test you’d like your patient to have, and use the button to submit.
